Description of Services: Supports clients and their caregivers at the end stage of life * practical, physical and emotional support to maintain comfort and quality of life for the client as a compliment to professional medical care

Services include:
  • case management
  • visiting volunteers
  • companionship
  • respite care
  • emotional support to people and their family/caregivers dealing with a life-threatening illness

Bereavement support for anyone who has experienced the death of someone close * services can be provided at home, or in hospitals and long-term care homes

Bereavement Services - Support groups held in various communities throughout the year
Requirements
Fees: Free
Eligibility - Population(s) Served: People who need end-of-life care or bereavement supports * must live within geographic service of hospice (Steeles Avenue to the Lakeshore, Yonge Street to Victoria Park) * 18 years of age or older
Application: Call * no referral required * referrals from health care professionals require completion of the Palliative Care Common Referral Form
