Description of Services:
  • Offers health services and a variety of health programs
  • Provides access to other health care professionals within and outside of the Family Health Team
  • Coordinates documentation if patient receives care from a member of the team
Administers the following programs and services:
  • Chronic Obstructive Pulmonary Disease (COPD) or Asthma Program
  • Diabetes Self-Management Program
  • 18-Month Well-Baby Visits
  • Hypertension Program
  • INR Management Program
  • Memory Clinic
Also provides access to services through the following community partners:
  • Ontario Health atHome - North West Area
  • Northwestern Health Unit
  • Community Counselling and Addiction Services
  • CAMH Centre for Addiction and Mental Health
