Description of Services: Provides services to individuals and families to ensure access to the necessary supports to be part of the community
  • Provides developmental screening and assessments
  • Offers links to health, financial, education and social services within the community
  • Facilitates recreational and life skills programming
  • Provides guidance during the transition to adulthood and independence

Eligibility - Population(s) Served: Children and youth 6-18 years of age with a developmental disability and their families
