Description of Services:	Provides services and supports to assist individuals to achieve and maintain a quality life Assists individual in developing a realistic plan to acquire the skills and supports to enable them to live independently Determines areas of need and develops the support plan with a focus on each individual Provides referrals to community resources and supports Provides the supports in a community setting or within individual's home environment Focuses on wellness and recovery Supports can include assistance in the following areas: Daily living skills, basic needs and life and social skills Employment Navigating the mental health system Accessing medical and psychiatric services Medication and symptom management Crisis prevention, intervention and management

Eligibility - Population(s) Served:	Individuals 18 years of age and older diagnosed with a serious mental illness Clients must voluntarily agree to services

Application:	Intake assessment required Support can be brief or long-term
