Description of Services:	Assists individuals to secure and maintain adequate housing Provides guidance and support with landlord issues Provides limited financial assistance to help pay (top off) rent Provides program participants with: Case management services Recovery-based focus Home management support

Eligibility - Population(s) Served:	Individuals 18 years of age and older who experience an on-going mental health disability or are assessed as in recovery from an addiction Restricted to Employment and Income Assistance Participants, ie recipients of Ontario Works (OW) or Ontario Disability Support Program (ODSP), or otherwise qualify for Rent Geared to Income (RGI)

Application:	Requirements include: Completed housing application and Intake assessment
