Description of Services:	Supports for individuals living with prediabetes or diabetes Provides education and services for the prevention, management and treatment of diabetes Provides case management and screening services Offers basic foot care Offers walking programs and exercise programs tailored to meet the needs of the individual Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes Advises on developing and implementing an approach to care and management of diabetes Assists individuals in finding appropriate services Provides referrals to other health care professionals
