| Description of Services: | Broker service, operated by CHAP, which matches clients to appropriately skilled Home Helpers, based on client-identified need and goals * home helpers are self-employed Independent Resource Contractors who have been carefully screened by the Home Help Service Coordinator * screening includes a criminal background check, verification of professional credentials, and professional reference checks * in-home assessment is offered free of charge to ensure all client needs are addressed * all assessments are completed by the Home Help Service Coordinator who is a registered healthcare professional * services offered range from general housekeeping and maintenance to personal support and attendant care * referrals are welcome from community partners, healthcare providers, family, friends, or self * to initiate a referral or inquire about services, please phone 519-354-6221 ext 246 or email chap@familyservicekent.com | ||
