Eligibility - Population(s) Served: Adults 65 years and older, and those younger than 65 if experiencing difficulties typically associated with aging (e.g. Parkinson's Disease, dementia) * must be a resident of Hamilton or of the larger Hamilton Niagara Brant Haldimand region if unable to access service close to home * must not be currently under the care of a geriatrician or geriatric psychiatrist elsewhere
Application: Medical referral required * patient/substitute decision-maker must consent to referral
