Eligibility - Population(s) Served: | Adults 18 years and older who are in the final stages of a life-threatening illness * must require the expertise of an on-site, palliative care interdisciplinary team to manage symptoms and distress * must have goals of care promoting a comfort approach rather than aggressive treatment * must have a life expectancy ranging from several hours up to several months duration * must have care needs that cannot be met at home or other settings with the available resources | ||
