Description of Services: Supports for people who have serious and persistent mental illness * clients typically have co-existing problems such as homelessness, substance use, or involvement with the judicial system
Requirements
Fees: Free
Eligibility - Population(s) Served: People 16 years and older who have a psychotic disorder such as schizophrenia or bipolar disorder with psychotic features * must be experiencing ongoing illness that affects several aspects of life * must have had multiple hospital admissions * must need treatment support provided in their home or community * must not have been successful with less intensive programming * must have a focus on recovery * must be able to work with a team of professionals
Application: Call * accepts self-referrals and referrals from family and friends, community agencies, and healthcare professionals
