Fees: Covered by OHIP
Eligibility - Population(s) Served: Adults 16 years and older who have sustained a concussion, with diagnosis based on one or more of the following: loss of consciousness lasting 30 minutes or less, post-traumatic amnesia that is less than 24 hours, and confusion or disorientation at the time of injury
Application: Referrals accepted from emergency department or urgent care of Hamilton Health Sciences and St Joseph's Healthcare Hamilton, and from family doctor/nurse practitioner/medical specialist within the Hamilton Niagara Haldimand Brant region
