Description of Services: GAIN provides comprehensive geriatric/senior assessments for those with multiple, complex health concerns.

GAIN addresses complex health concerns such as:
  • memory loss or confused thinking
  • walking and mobility
  • falls or risk of falls
  • bladder or bowel control
  • multiple medications
  • difficulty coping
  • safety issues
  • frequent hospital visits

Eligibility - Population(s) Served: Seniors who live at home or in retirement residences.
