Description of Services: Community based GAIN clinic provides comprehensive geriatric assessments and care planning and delivery to address the complex needs of many older adults. The inter-professional teams work with older people and their families to develop care plans that support frail seniors with complex health issues so they can remain living at home * Staff include:

Eligibility - Population(s) Served: Older frail seniors, aged 65 years and older, with related complex health issues
Application: Physician or nurse practitioner referral required * self-referrals accepted
