Description of Services: Comprehensive services for clients whose enrollment extends beyond three months * program includes individualized assessment and care planning that meets client's unique needs * case manager facilitates access to necessary resources and services * clients receive ongoing supportive counseling to help manage their challenges and are connected to appropriate community or healthcare services * clients could be referred to:

  • Sahara Addictions Day Program
  • Sahara Men’s Group
  • Sahara Relapse Prevention Group
  • Sahara Opioids Prevention Support Group
  • Intensive Addiction Day Treatment Group
  • Family Group

Eligibility - Population(s) Served: People 16 years and older with an addiction issue
Primarily South Asian
Application: Self referral, physician referral or professional agency referral
