Description of Services: Provides in-home and information and referral services by care coordinators in the Peterborough Regional Health Centre.

Hospital care coordinators assess and arrange for required Ontario Health atHome in-home care services upon discharge from hospital and links patients to other useful community resources.

In-home services may include personal care, nursing, physiotherapy, occupational therapy, speech language therapy, dietetic services and social work services.
Requirements
Fees: Covered by OHIP
Eligibility - Population(s) Served: Must be a resident of Ontario with a valid Ontario Health Insurance Plan (OHIP) card. Must be a patient of Peterborough Regional Health Centre. Further eligibility will be assessed upon initial intake according to the Long Term Care Act 1994.
Application: Although initial inquiry may come from patient/family member, the formal referral must come through a hospital professional such as a physician, social worker or nurse.
Waiting Period - Case management services are available prior to discharge from hospital
