Description of Services: Ontario Health atHome assess and coordinate access to professional health care services such as nursing, physiotherapy, occupational therapy, speech-language therapy and dietetic services for children with special health needs who require assistance in the school setting.

Fees: Covered by OHIP
Eligibility - Population(s) Served: Services are available province-wide to children 21 years of age or younger with special needs in a school registered with the Ministry of Education and whom are Ontario residents insured under the Ontario Health Insurance Plan (OHIP). Additional eligibility criteria will be assessed upon initial intake.
Application: Call in to the office to speak to a School Health Support Services Care Coordiantor to make a referral.
Waiting Period - No waiting period to make a new referral. Waiting periods for direct service vary depending on type of service required and need.
