Supports for people living with prediabetes or diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff * monthly diabetes education groups offered virtually
The diabetes educator is responsible for assessing health needs, education and ongoing management for clients with type 2 diabetes only To access the Centre for Complex Diabetes Care program at Lakeridge Health, call the Ontario Health atHome Central Diabetes Intake at 1-888-997-9996 to speak with an Ontario Health atHome Care Coordinator who can assist in the process, or fax the completed form to 1-905-444-2544 |
