Description of Services: Community Support workers offer long term case management to clients in WoodGreen housing sites or to clients living independently. Clients living in WoodGreen buildings are required to have an in-person visit with a worker, once a month. Clients living independently are required to have an in-person visit with a support worker, once every six months.
Requirements
Application: Referrals are accepted from anyone, including self-referrals. Referrals are through The Access Point. Please call or fill out the application form online. Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment. Once referred to Woodgreen, the housing manager will arrange an initial interview, followed by a tour or the residence for acceptable applicant. Eligibility Criteria: Individuals who have experienced homelessness and are recovering from mental health challenges. Must be able to live independently. Individuals may also contact WoodGreen Housing and Homelessness Services.
