Program: Dual Diagnosis Case Management Program - Toronto - Lawrence Avenue

Description of Services: Provides individualized support to clients living with a co-occurring developmental disability and a diagnosis of serious mental illness. The team also provides dedicated support to individuals with a dual diagnosis who also have justice system involvement. The supports offered to clients typically include: comprehensive assessment, collaborative development of a personalized safety plan, collaborative development and implementation of an individualized recovery/goal plan and assistance to achieve/maintain optimum health and wellness.

Eligibility - Population(s) Served: Dual Diagnosis Only
Application: For individuals with justice system involvement, referrals must be from designated priority referral sources which include Mental Health Court Support Services, Police, Probation/Parole, the Law and Mental Health program at CAMH, Ontario Shores Centre for Mental Health Sciences' Forensic Unit and other funded Mental Health and Justice services.
