Description of Services: Hospital-based outpatient rehabilitation for adults who have had a stroke, acquired brain injury, spinal cord injury, or have complex musculoskeletal conditions * provides assessment and time-limited treatment to:
  • maximize recovery from a recent acute event
  • maximize functional abilities and patient safety
  • promote adaption of/to home
  • support transition to appropriate community-based programing
  • support timely transition from acute care to inpatient rehab setting
  • prevent admission or re-admission to an acute or rehab hospital
  • provide opportunity to learn/practice in a familiar, stimulating, and supportive environment
  • provide goal-driven treatment based on measurable progress and functional change
