Description of Services: Team of geriatricians, case managers, and nurses provide assessment, case management, and follow-up services to older adults who are experiencing any of the following:
  • confusion
  • memory concerns
  • functional decline and frailty
  • falls
  • bone loss/fragility
  • mobility issues
  • multiple emergency room visits

Home visits available for housebound patients only
Requirements
Fees: Most services covered by OHIP
Eligibility - Population(s) Served: Adults 65 years and older presenting aging-related difficulties
Application: Medical referral required * patient/substitute decision-maker must consent to referral
